Overall Goal for Wood Buffalo PCN

The overall goal for the Wood Buffalo PCN is to improve patient care, increase efficiencies and allow physicians within the Network to better coordinate and integrate their work.

Primary Objectives

The following primary objectives match those of the provincial primary care initiative.

  • To increase the number of Albertans with ready access to primary care.
  • To manage access to appropriate twenty four hour a day primary care services.
  • To increase the emphasis on health promotion, disease and injury prevention, care of patients with medically complex problems and care of patients with chronic diseases.
  • To improve coordination of primary health services with other health care services including hospitals, long-term care and specialty care services.
  • To foster a team approach to providing primary health care.

Secondary Objectives

The following, called secondary objectives, are those related directly to the Wood Buffalo Primary Care Network.  Each PCN in Alberta follows the provincial PCI objectives in their planning but individual network objectives may differ based on unique community needs.

  • To develop means by which physicians recruited to the region are retained in the community and engaged to work together.
  • To increase access for after hours primary care and unattached patients.
  • To enhance primary care for the patient with chronic disease and complex issues.
  • To improve and promote linkages between specialists and family physicians.
  • To utilize inter-disciplinary teams in PCN initiatives and service delivery models.
