What are the benefits to an individual in Primary Care Networks?

Once Primary Care Networks are established they should:

  • Help individuals find access to a family physician.
  • Coordinate care between the physician’s office and other health care services so it is more efficient and effective for patients.
  • Help individuals find care outside of usual clinic hours.
  • Assist physicians in working closer with other health care professionals so patients have access to a wide range of services.
  • Increase focus on health promotion, chronic disease prevention and care of those with chronic disease.
