Who provides health care services in a Primary Care Network?
The health region and family physicians work together in a Primary Care Network to provide care. A Primary Care Network assists family physicians in linking with other health care professionals in forming an inter-disciplinary team to provide comprehensive care to individuals.
A Network is considered an extension of the family physicians office with a range of other health care professionals providing services. For example, a diabetic patient may see a pharmacist to review medications and a dietitian to modify eating habits. The health region will also be involved through services such as home care, public health, hospitals and long-term care facilities.
