The WBPCN is intended to increase efficiencies and access to health care services.  Individuals continue making appointments with their family physician for health concerns.  The centralized site functions as an extension of the family physician’s office and provides up to 10 different health care professionals to whom the physician can utilize for team based, comprehensive, and coordinated care.  The referral is made through faxed communication from the family physician.  The WBPCN also attempts to provide seamless care between the health care center and the physician office within the identified program areas.
