Diabetes Program

Diabetes ProgramThis program is intended for individuals who have diabetes or who are at risk for diabetes.  There is access to an interdisciplinary health care team comprised of a registered nurse, pharmacist, dietitian, exercise specialist, mental health coordinator,  and certified diabetes educator.

  1. The diabetes program provides interdisciplinary, comprehensive care and includes the following supporting programs:
  2. - Pediatric Type I
    - Adult Type I and Type II
    - Gestational Diabetes Education
    - Diabetic Nephropathy Prevention Clinic
    - Tele-Ophthalmology

Referrals to the program are made through a family physician.
