Diabetic Nephropathy Prevention Clinic (DNPC)
Diabetic Nephropathy is also known as kidney disease or damage to the kidneys as a result of diabetes. The DNPC provides direct access to either an Endocrinologist (a physician who specializes in diagnosing and treating hormone imbalances) or Nephrologist (internal medicine physician who specializes in kidney disease). Every 6 months one of the specialists visit the WBPCN centralized site to see patients. In addition, patients have access to regular visits to the WBPCN to monitor health status with blood work, if requred. Clinicians will communicate health status and management plans with the patient’s family physician.
To qualify for the clinic patients have diabetes and high blood pressure (BP>130/80) or diabetes and proteinuris (protein in the urine). Patients have to be referred by their family physician to access the clinic.
