Pediatric Type I
In November 2009 the Wood Buffalo Primary Care Network launched a pediatric diabetes program for families and children from birth to 18 years of age. Newly diagnosed patients are referred by their family physician to the PCN clinic site team for care and continued follow up. An interdisciplinary team comprised of a nurse, pharmacist, diabetes educator, dietitian, exercise specialist, social worker, and pediatrician work closely together to provide seamless care. Key aspects of the program include local care, educating patients and families, and individualized follow-up. The program eliminates previous hardships related to travel to Edmonton for appointments, especially when a child is newly diagnosed and the family has to travel often to establish care.
Families that have previously travelled to the Stollery for care will be transferred to the PCN for local care. A release of information form is completed and faxed to the PCN and the child’s pediatrician. The PCN will call the family to arrange next appointment. This process is not mandatory. Families have a choice to remain going to the Stollery for appointments.
