Family Practice Nurse Program

Established in 2008 as a pilot, the Family Practice Nurse (FPN) Program extends primary care from the centralized clinic site to the family physician office whereby a nurse works collaboratively with a local family physician in their office setting during the week. Presently, there are 6 nurses working 1 to 2 days a week in the office of 9 family physicians. Communication between the physician and nurse is vital for effective, collaborative care within the FPN program. Both the physician and nurse work to establish an open and flexible relationship.

 Working to their full scope of practice with physicians, the FPN nurses:

  • Provide patient education and counseling
  • Increase patient access to primary care         
  • Provide chronic disease management (CDM)
  • Provide patient follow-up

Evaluation of the FPN Program takes place on a regular basis with physicians and nurses and patient feedback is gathered from patients with the intent to make changes that improve interdisciplinary care within the family physician office.
