Geriatric Care

Geriatric CareThis program is intended for individuals who are aged 65 and over requiring health care services for physical, medical, cognitive and social concerns that limit independence and quality of life.

Seniors have access to an interdisciplinary team of health care professionals who work together to provide two vital health care services:

 Adult Day Program
 Fall Prevention

Family physicians can refer individuals to the Adult Day Program.  Individual’s can self-refer to Fall Prevention by calling the WBPCN directly.
