Heart Health Program

Heart Health ProgramThis program is intended for individuals who have heart disease or who are at risk of heart disease.  There is access to an interdisciplinary health care team comprised of a pharmacist, registered nurse, dietitian, and mental health coordinator.   Workshops include Cholesterol: Be True to Your Heart, High Blood Pressure: Get the Low Down, Success Over Stress.

This program includes the following aspects:
- A team of health care professionals who work together with your family physician
- Linkages with community resources and local hospital
- Cardiac rehabilitation: direction and education in areas of diet, exercise, medications, grieving, return to work, and stress
- Flexibility around your schedule
- Continued collaboration with family physician

Smoking cessation counseling is a component of this program.

Referrals to the program are made through a family physician.
