This program is part of the core Heart Health Program and intended for individuals who have had a cardiac event (i.e., heart attack, bypass surgery, angina, angioplasty). There is access to an interdisciplinary health care team for a 5 week group cardiac rehabilitation program – an exercise and education program. The education component is a workshop called Change of Heart with aspects taught by the registered nurse, exercise specialist, and mental health coordinator.

Referrals to the program are made through a family physician.
