Palliative Care
NEW Please be advised that the palliative care program is currently being restructured as there will no longer be a palliative care clinic. Any new information will be added to accordingly.
This program is intended for individuals of adult age (18 and older) who have been diagnosed with a life limiting illness. There is access to an interdisciplinary health care team who work together an individual’s family physician. The health care team includes a registered nurse, pharmacist, occupational therapist, and mental health coordinator who provide support in connecting to community health services and resources.
Referrals to the program are made through a family physician.
