This program is intended for individuals who are recovering from stroke or who are at high risk of stroke.  There is access to a team of health care professionals, educational workshop and the Stroke Clinic. The education component is a workshop called Change of Heart.

Stroke Clinic – The stroke clinic provides individualized care by a health care professional for those recovering from stroke or TIA (min-stroke) or those at high risk of stroke.

Referrals to the stroke program are made through a family physician.
